Note: All forms must be typed and submitted to the Office of Student Engagement in person. Mailed or e-mailed submissions will not be accepted.

Form Name Description When do I use it?
Stipulation Validation Form This form is used to report the completion of a stipulation event. All organizations that receive SOFAC funding must complete one stipulation event per fiscal year. This must be submitted within 10 business days of the event’s end date. Use this form when you have completed your stipulation event.
Trip Registration Form This form is used to list all the students attending an event that is 10 or miles outside of Springfield. This must be submitted at least 10 business days prior to the event date. Failure to submit this form will result in a loss of funds for that event. Use this form when you are traveling 10 or more miles outside of Springfield for an event.
Organization Trip & Activity Release Form This form is used as a release for each student attending an event that is 10 or more miles outside of Springfield. Each student traveling must complete this form. This is attached to the Trip Registration Form. This must be submitted at least 10 business days prior to the event date. Failure to submit this form will result in a loss of funds for that event. Use this form when you are traveling 10 or more miles outside of Springfield for an event.
